Once this form is submitted, you will be receiving a prepaid return shipping label with a return address of 9220 Winnetka Ave, Chatsworth, CA 91311.
Please use this return shipping label to return the entire unopened product, or if your product has been opened it is acceptable to return just the factory sealed resin container. Unsealed resin containers are outside the scope of this recall.
This recall allows for you to select whether you prefer a replacement of equal value, or a refund. Please allow up to 12 weeks from return date of your product to receive a replacement or a refund.
NOTICE OF RECALL OF BRATZ™ MAKEUP DESIGN SKETCH BOOK
MGA Entertainment Inc. is recalling 6,200 units of the Bratz Makeup Design Sketch Book (Item #504788). This product has the potential to be contaminated with Staphylococcus warneri and Staphylococcus intermedius. MGA Entertainment is recalling products produced between November 5, 2010 and February 25, 2011. MGA Entertainment has ceased production and distribution of this product.
NOTICE OF PARTIAL RECALL OF BRATZ™ STYLIN' SCOOTER
Any customer who purchased a Bratz™ Stylin’ Scooter (Item # 266563), manufactured for MGA by Jurong Dumar Bicycle, Inc. (“Dumar”) prior to July 2004, should immediately stop using the scooter and notify MGA. The wheels of these scooters may present a risk of injury to children riding the scooter.
